Here is a close-up of the teeth on the bad hub. This is an extreme case. A synchro hub doesn't have to be so badly worn to be unusable, but human nature being what it is, most people won't pull the transmission and rebuild it until the shifting has become intolerable, and by then the teeth might well be this bad. Any significant rounding of the teeth warrants replacement. The synchro bands and tooth rings are critical to the smooth operation of the transmission, so they should be replaced if there is any question about their usefulness.
